You can check www.tripadvisor.in to connect with local tour operators. Actually .in is for India, so do find the right site for Nepal and I am sure that there is no Tripadvisor for Tibet (China's restriction)

I would recommend Third Eye Travel, we used them to go to Nepal, they also can do Tibet, contact them with your specifics and I'm sure them can come up with a good itinerary.
